The HystSim is a world first that brings obstetric and gynaecology surgeons the most advanced training system for diagnostic and therapeutic hysteroscopy available. Already, medical research has found it to provide “highly realistic and useful training for hysteroscopy surgery with high acceptance ratings for both experienced and novice surgeons.” read more

HystSim’s diagnostic hysteroscopy module offers 12 virtual patients with different pathologies and varying levels of difficulty. The module gives trainees experience in the use of angled optics and trains them to establish a clear view and visualise the entire cavity.
The practice opportunity is supplemented by a comprehensive performance review which provides feedback on how much (and which parts) of the uterine surface has been visualised, competency of fluid handling, and parameters such as intervention time and number of collisions with the uterine wall, factors which effect patient comfort and safety.
Trainee objectives:
• To establish uterine distension and clear viewing conditions
• To confirm the correctly placed hysteroscope by identifying
the right and left tubal orifice
• To inspect the uterine cavity completely by directing the
camera efficiently over the entire endometrial surface while
maintaining clear view
• To describe all visible pathologies
URO HystSim features four different virtual-
Easy diagnostic cases and small polyps provide an ideal starting point for novice surgeons, whilst challenging fibroids allow more experienced surgeons to try new techniques and to maintain surgical proficiency. There is also the option for instructors or trainees themselves to start complications and emergency scenarios in order to hone performance in these situations.

Polyp Removal is the first of HystSim’s therapeutic hysteroscopy modules. The module offers 8 virtual patients with polyps of different sizes in different positions. The module provides training for the first steps in operative hysteroscopy using the loop electrode.
Performance review provides feedback on the amount of the polyp removed along with parameters relating to intervention time, efficiency of movement and safety levels.
Trainee objectives:
• To inspect the cavity completely and to describe visible
pathologies
• To cut the polyp with the cutting loop. If the polyp is not larger
than the hysteroscopic sheath, try to cut the stalk directly. If it
is larger, cut it in chips
• To remove the stalk completely while preserving the healthy
Tissue
• To do a safety inspection at the end of the procedure

The Myomectomy module on HystSim features 8 virtual patients with varying types of submucosal fibroids (type 0), all in different positions and with different levels of difficulty.
Performance review provides feedback on the amount and quality of the removed fibroids along with parameters relating to intervention time, efficiency of movement and safety levels.
Trainee objectives:
• To inspect the cavity completely and to describe visible
pathologies
• To cut tmyoma in chips with safe handling of the loop
electrode
• To remove cut pieces from the cavity
• To coagulate bleeding sources
• To do a safety inspection at the end of the procedure

Despite an array of newer ablation techniques, rollerball endometrial ablation remains a popular choice albeit one which needs more training.
HystSim’s endometrial ablation module contains 4 different virtual patients with varying shapes of uterine cavities for full procedural practice. Endometrial ablation with the rollerball is also an ideal exercise to gain practice in electrosurgery in all positions and in the entire uterus.
Performance feedback provides a unique visual overview of the coagulated uterine surface as well as all the parameters relating to intervention time, efficiency of movement and safety levels.
Trainee objectives:
• To inspect the cavity completely and to describe visible
pathologies
• To use the rollerball in a systematic way to ablate the complete
endometrial surface while not ablating the endocervix
• To do a safety inspection at the end of the procedure

To accompany full hysteroscopy procedures, HystSim also simulates all relevant complications
which can occur during the hysteroscopy procedure -
The realistic simulation of bleeding requires trainees to detect and coagulate the source of the bleeding whilst using fluid handling to maintain a clear view.
Perforation of the uterine cavity is one of the most common hysteroscopy surgical complications and instances of this, along with the response, will be recorded in the trainee’s feedback report.
The “instructor control” function allows instructors to create complications at the click of a button in order to challenge trainees. These range from bleeding and perforation to instrument failure and fluid overflow, and are designed to teach trainees the correct responses as well as to avoid panic if these situations arise during a real procedure.

HystSim uses a real resectoscope to enable familiarity with instrument handling during
the hysteroscopy, this includes foot pedals for electrics and coagulation and valves
for fluid handling as in genuine procedures. Tools such as rollerball are loop electrode
are realistically simulated and viewed on screen exactly as would be the case in

Whether the HystSim is used for training, evaluation, studies or assessment, the inbuilt login for administrators and instructors is key to its versatility.
This mode facilitates the following:
• Creation and organisation of new users
• An optional guest admin account
• Tracking and assessing trainees development of skills
and improvement over time -
• Generating extensive statistics of an individual or group performance, which can also be used for studies
and future certification
